Event Attendee Waiver

I am aware of the risks of the novel coronavirus (COVID-19) and the current guidelines and recommendations of the Centers for Disease Control (CDC), the Pennsylvania Department of Health and the City of Philadelphia (“Guidelines”).

By entering this venue and participating in this event, I, on behalf of myself and any minor children or dependents of mine attending this event, and our respective personal representatives and heirs, voluntarily assume all risks related to COVID-19 exposure or infection, and forever waive, release and hold harmless Catering By Design, and their respective affiliates, members, directors, officers, employees and agents and event venue, from any claims, losses or damages of any kind arising from or relating to such risks.

I agree that I will not attend the event nor enter the event premises if I

• am experiencing or have experienced symptoms commonly associated with COVID-19 on the day of the event or within 14 days prior to the event
• am diagnosed with COVID-19 within 14 days prior to the event
• have contact or live with someone infected with or experiencing symptoms commonly associated with COVID-19 within 14 days of the event
• have a temperature of 100.4 or higher

I agree to practice safe gathering practices as required and recommended in the Guidelines, and encouraged by the facility, including masking at all times (except when eating or drinking), safe distancing and frequent hand washing and sanitizing.
